Texas Department of Aging and Disability Services Engages with AxisPoint Health for Coordinated Care Management Services

Comprehensive Program to Improve Quality of Life for Individuals with
Intellectual Disabilities

other risk-bearing entities, today announced that the Texas
Department of Aging and Disability Services
(DADS) has contracted
with the company to provide coordinated care management services to
individuals with intellectual disabilities residing in state supported
living centers (SSLC), as well as to individuals who have recently
transitioned from an SSLC into the community. The individual-focused
program is designed to improve the overall health and quality of life
for this unique population.

AxisPoint Health will provide services to over 3,000 individuals across
the state with wide-ranging and complex care needs. AxisPoint Health
will help DADS identify health goals and monitor outcomes for SSLC
residents, proactively track quality metrics, and develop and implement
personalized care management plans for individuals who have transitioned
into the community. Further, AxisPoint Health will conduct necessary
community outreach; raising general awareness, educating professional
associations and engaging key stakeholders.
